Vaccine card or old pet passport that contains Rabies vaccination history. If you have neither, please ask us to check whether we have evidence in your clinical records or have your previous vet email it to us at Hello@theealingvet.co.uk
The pet(s) who are travelling.
The owner/person who is travelling with the pets — you will be required to sign a declaration. Please do not sign unless the vet has checked you are signing in the correct location and with a BLUE pen provided by the vet.

Please note: All Animal Health Certificate (AHC) appointments must be booked at least 10 days before your planned travel date. Appointments booked with less than 10 days’ notice may incur an urgent appointment fee.

I understand that the information I provide must be accurate and that if it is not, the AHC may be incorrect, and I may be refused entry to the EU at border control. I accept that it is my responsibility to ensure the information entered above is final. I understand that if any changes are later made to any of the details in this form that the vet will need to produce a new AHC and that in this case I would be liable for a second AHC fee.
